In the New Patient visit it is filled LAST, after the consent stack (new_patient_consents) and the MRS before-HRT symptom review (physician 2026-10-02). The MRS symptom grid now lives on mrs_before_hrt and the onboarding consents on new_patient_consents — neither is embedded here anymore.
