The new-patient onboarding consent stack (Privacy Practices, Medical Care, Telemedicine, Lab Orders, HIPAA Authorization), split out of the New Patient packet so ALL consents are signed at the FRONT of the visit before the symptom review and the medical intake sheet (physician 2026-10-02). Sexless — bundled with both the male and female New Patient groupings. A short identity section captures name + DOB first so the consent name-match works; it dedups away for an already-identified patient.
